Membership Application and package

Annual membership from January 1, thru December 31 of same year. Annual dues are $25.00 *If you want your membership package mailed to you, please add $ 1.50 for postage. (packages will not be mailed if postage is not included).

Membership Package
All members will receive two 10-13 membership cards and one window decal.
Additionally member will receive a full color 8in.x 6in. Laminated vehicle *ID card.
If membership pkg. picked up at meeting $25.00 or $26.50 if mailed.
If you want your membership package mailed please add $1.50 to cover postage.

* Please note for control purposes, please send a photo copy of vehicle registration certificate for each vehicle registered to member, or bring certificate to the next general meeting. (ADDITIONAL VEHICLE ID CARDS WILL ONLY BE ISSUED TO VEHICLES REGISTERED TO THE MEMBERS NAME ONLY. ADDITIONAL CARDS ARE $5.00 EACH PLUS $1.50 FOR MAILING.) In no case will any member be issued more then (3) Three Vehicle ID Cards.(Your Original Plus up to two additional) Note. The ID Cards are the Property of Brooklyn / Staten Island 10-13 Association Inc. and are to be used to identify a members vehicle at our meeting locations. Any Misuse of the Card could result in removal of the card.

**Due to the US Department of Homeland Security "High Alert" and elevated threat level in NYC, members vehicle ID cards will not be distributed to Non-Law Enforcement Personnel.
* NEW APPLICANTS MUST SEND PHOTO COPY OF POLICE ID ALONG WITH THE MAIL IN APPLICATION IF YOU WANT YOUR VEHICLE ID PLAQUE MAILED TO YOU. (Law Enforcement Personnel includes both Active & Retired M.O.S. as well as current Active members of the Bk-SI 10-13 Organization.)

Include $1.00 for postage for each 10 items ordered. Mixed or all the same.
